Overview
The American Academy of Pediatrics Pennsylvania Chapter is sponsoring the Every Child Deserves a Medical Home Training Program in Pittsburgh, PA September 15, 2001. The program is also being hosted by many organizations in Pittsburgh and is not officially managed by the American Academy of Pediatrics National Office. The program aims to support pediatric health care providers, children with special health care needs and their families, and communities interested in the well being of these special needs children in a changing health care environment.

Target Audience
The target audience includes pediatric health professionals and their staff, other professionals, families of children with special needs, dentists, managed care professionals, policy makers, community members, and and other child advocates who care for children with special needs.

Cost

  • $50 for Physicians & Dentists
  • $40 for Other Professionals
  • $25 for Parents and Caretakers

Course Objectives
General pediatricians, family physicians, pediatric subspecialists, and other pediatric health care professionals, nurses, pediatric office staff, community resources, and parents of children with special needs are invited to participate in the course. Upon completion of this course, participants will be able to:

  • Explain the elements of the medical home concept as applied to their practice environment or child's care;
  • Understand the concepts, skills, and information necessary to successfully care for children with special health care needs who are enrolled in managed care organizations;
  • Accurately describe trends and developments in caring for children with special health care needs;
  • Be able to identify programs in the community that serve children with special health care needs; and
  • Be able to assess, and if necessary improve pediatric office practices to assure that they are sensitive to families of children with special health care needs.

I). Family/Professional Partnerships
The common elements of family centered care and Strategies to develop family-professional partnerships will be discussed.
II). Practices, Policies, and Procedures
Practical strategies to implement medical homes into practice will be discussed.
III). Comprehensive, Coordinated, Collaborative Care
Strategies for coordinating care, working with community based resources, and collaborating with specialists and allied health care professionals will be discussed.
IV). Transitioning Children and Youth to Adulthood
Understand transition related issues and strategies for advancing children and youth into adulthood

All participants will receive a training manual, break food and lunch.
On-site daycare is not available.
Parent scholarships will be awarded on a first-come, first-serve basis.
