Overview
The American Academy of Pediatrics (AAP) and Shriners Hospitals for Children
in collaboration with Baystate Medical Center Children's Hospital will offer
its Every Child Deserves a Medical Home Training Program in Massachusetts
on November 8, 2003. The program aims to support pediatric health care providers,
children with special health care needs and their families, and communities
interested in the well being of these special needs children in a changing
health care environment.
Target
Audience
The target audience includes pediatric health professionals
and their staff, other professionals, families of children
with special needs, managed care
professionals, policy makers, community members, and and other child advocates
who care for children with special needs.
Course
Objectives
General pediatricians, family physicians, pediatric
subspecialists, and other pediatric health care professionals,
nurses, pediatric office staff, community
resources, and parents of children with special needs are invited to participate
in the course. Upon completion of this course, participants will be able to:
- Explain the elements of the medical home concept
as applied to their practice environment or child's
care;
- Discuss practical strategies for helping parents,
providers, and young people transition to adult independence,
health care with funding, and work;
- Describe practical strategies to translate the
concept of Medical Home for CSHCN into office or
clinic practices sensitive to the needs of CSHCN
and their families;
- Identify resources available to CSHCN and their
families and strategies for coordinating care and
promoting interagency collaboration;
- Discuss current issues in managed care coverage
and reimbursement as they relate to the Medical Home
for CSHCN and transition to adult health care.

Common Elements
This opening session provides an overview of the medical home approach
to quality, comprehensive health care for children with special needs,
and begin to outline the ethical principles involved with this pediatric
approach.

A. Practices, Policies and Procedures
Practical strategies to implement medical homes into practice will be discussed.
B. Family-Professional Partnership
The common elements of family-centered care and strategies to develop family-professional
partnerships will be discussed.
C. Transitioning Children and Youth to Adulthood
Understanding transition related issues and strategies for advancing children
and youth to adulthood.
D. Comprehensive, Coordinated, Collaborative Care
Strategies for coordinating care, working with community based resources,
and collaborating with specialists and allied health professionals will
be discussed.
E. Surveillance and Screening
This session discusses the importance of constant surveillance within the
medical home and suggests possible screening tools to use if surveillance
indicates.

Getting Oranized Workshop
A workshop held on how to best organize all the forms, reports and other
documents that office staff and parents/caregivers receive and share.

Western MA and the medical home
A panel discussion would be offered to discuss what is currently happening
regarding care for children with special health care needs and their families
in Massachusetts. Representatives from key systems (Title V, advocate,
legislator, Medicaid, family, etc.) would be present what services his/her
system is currently engaged in related to CSHCN, along with plans for the
future. A Q/A session would follow.
