Webinar—Oral Health Risk Assessment in the Medical Home
Through this webinar the faculty will discuss how to implement oral health services in the medical home. It will include instruction on how to use the AAP Oral Health Risk Assessment Tool to identify children at risk for early childhood caries (cavities) and using oral health as a topic for quality improvement, information on oral health anticipatory guidance including diet, oral hygiene, and fluoride use, and practical tips on how to implement oral health services in the medical home from getting paid to the application of fluoride varnish to finding a dentist to see your patients.

Workshop—URAC Patient Centered Health Care Home (PCHCH) Practice Achievement

The URAC PCHCH Practice Achievement Program evaluates practices on 28 key URAC PCHCH Practice Standards from the URAC PCHCH Program Toolkit. These standards are considered essential elements of a patient centered health care home and include 7 mandatory standards. The essential standards align to the Joint Principles of the Patient Centered Medical Home and address key requirements for all Meaningful Use requirements for electronic medical records, e-prescribing, and quality data submission.

The 5th International Conference on Patient- and Family-Centered Care
This conference, hosted by the Institute for Patient- and Family-Centered Care, will provide opportunities to address emerging issues in patient- and family-centered care, both at the systems and individual levels. It will include presentations from hospitals, primary care, specialty ambulatory settings, community-based programs, public health and mental health agencies, patient- and family-led organizations, schools of medicine, nursing, and other health and helping professions. The conference will also showcase videos developed by health care organizations to support partnerships with patients and families and will feature a variety of formal and informal meetings, including plenary sessions, concurrent/topical breakout sessions, networking discussions, video theatre, and poster presentations.

In this Patient-Centered Primary Care Collaborative webinar, faculty will explore the definition of the care team and care coordination as well as the key elements of care coordination within the patient-centered medical home (PCMH). Faculty will also talk about the patient’s perspective by reviewing Christine Bechtel’s research on patients and the delivery system as a whole—its challenges and potential solutions—including care coordination and the medical home.
