information concerning their infants, child’s, or
adolescent’s nutrition as well as resources for nutrition
information. Also available in Spanish.

This information was adapted from Bright Futures in Practice:
Nutrition and is organized by age to provide families with
all the latest information concerning nutrition and issues
related to it. This can help families discuss with health
professionals issues and concerns they have about their
child’s nutrition.
Bright Futures Encounter Forms for Families
Description:
Bright Futures Encounter Forms for Families are forms that
outline what to expect at a given well-child visit as well
as suggested developmentally appropriate topics that parents
may want to discuss with their pediatrician. Also available
in Spanish.

way. It promotes great discussions while encouraging reading,
coloring, drawing, and filling in the blanks.
Bright Futures: What to Expect
and When to Seek Help
A set of Bright Futures tools designed to help families
and service providers from a range of disciplines support
the healthy social and emotional development of children
and adolescents.
Product Highlights:
The tools are based on Bright Futures in Practice: Mental
Health, the four developmental tools offer framework
for providers and families to begin a conversation together
about how best to support healthy social and emotional development
in children and teens.
Bright Futures Family Tip Sheets
Description:
The Bright Futures Family Tip Sheets are divided into the
four stages of development: infancy, early childhood, middle
childhood, and adolescence. For each stage, there are 6
pages of information on topics such as health professional
visits, social development, safety and physical activity,
as well as information specific to the developmental stage.
Product Highlights:
This information can provide families with important and
easy-to-read information on their children’s health
as well as increase awareness of developmental changes children
experience as they grow.
Bright Futures Health Record
Description:
Bright Futures Health Record is a convenient form for parents
to record information about a child’s growth and development
and immunizations from birth through age 6. Also available
in Spanish.
Product Highlights:
This health record also lists issues that the family may
want to discuss with their health professional in a family
friendly format.
