The Medical Home Promising Practices Forum, June 24-25 2008

Overview
In June 2008, the eight MCHB Medical Home Grantees gathered in Bethesda, MA to highlight their project successes in medical home implementation over the past four years (2004-2008). This Forum was structured around the following medical home core elements: family-professional partnership, community-based systems (i.e., care coordination), transitions, and value (this encompasses reimbursement/appropriate financing).  Click here for Forum Agenda.
 
The eight Grantees were from: Illinois, New Hampshire, New York, Ohio, Massachusetts, Pennsylvania, Florida, and Wisconsin. 

This page provides links to each of the Grantees presentations at the Forum, and other information that may be useful for communities trying to implement medical home for children and youth with special health care needs.

Participants:
The eight Grantees; Representatives from the National Center of Medical Home Initiatives for Children with Special Needs, National Center for Cultural Competence, National Center for Childhood Hearing and Management, Catalyst Center, Family Voices, ChampionsINC, Healthy and Ready to Work, Evidence-Based Center, Association of Maternal and Child Health Programs (AMCHP), American College of Medical Genetic Coordinating Center (ACMG), family and young adult leaders, state and federal Title V leaders, physicians, and other health care providers. 

Next Steps:
In an effort to build the evidence base for medical home, a Writing Commitee is currently working on a final product to be disseminated from the Forum.
