The
Road to Medical Home:
Which path are you taking?
The National Center of Medical Home Initiatives for CSHCN
held the annual medical home grantee meeting on Sunday,
July 18th at the Chicago Sheraton Towers. During this
one-day meeting, participants had the opportunity to network
with medical home state, community, and practice teams,
learn how to sustain quality improvement initiatives,
and establish collaborations with critical partners. Updates
form the Medical Home Mentorship Network were provided
as well as the opportunity to share lessons learned on
how to incentivize practices to increase their medical
homeness? and how to sustain/spread medical home initiatives.

Guest List:
In addition to current medical home grantees, we added to
our guest list past medical home grantees and practice participants
of the National Medical Home Learning Collaborative. It
is our goal to continue to expand our efforts and knowledge
base on how states, communities and practices are improving
access to medical homes and improving the lives of CYSHCN
and their families.

medical homes at the state, community, and practice level.
Medical home teams/practices provided strategies and lessons
learned while a panel of experts also provided feedback.
